Complaints And Concerns
Patient feedback is a key part of our ongoing continuous improvement program. This valuable information can be used to improve patient experiences and to ensure a healthy work environment for both patients and staff. If you have any concerns, please follow our process below. We appreciate your feedback.
How to contact us with your concerns
You can deliver you concerns by phone, email or fax or you can ask to speak with one of our managers at any time.

What to Include:
- Your full name and contact information
- Location where care was received
- Type of service that was performed (x-ray, ultrasound, ECG or stress test)
- Date of occurrence
- Short explanation of what happened
- Expectations for follow up
What to Expect:
- Every concern will be promptly investigated by a manager
- Upon receipt of the complaint; a manager will contact you within 5 business days
- All concerns and complaints will be reviewed with our Medical Director
- Once investigated, a manager will follow up with you within 10 business days
- A record of your complaint will be kept at the facility and may be used for quality improvement purposes
- Patient confidentiality will be strictly adhered to during the entire process
